What’s normal after cataract surgery?
After cataract surgery, your eyes enter a natural healing period that often brings temporary discomfort and vision changes. These symptoms are part of the recovery process and often improve within days to weeks after treatment.
- Scratchy or itchy sensation: An itchy feeling is normal as you heal from the minor irritation during surgery.
- Light sensitivity: Your pupil takes several days to adjust after surgery, so glare from screens and overhead lights may be bothersome.
- Blurry or fluctuating vision: Vision fluctuates immediately after treatment but improves within weeks as swelling subsides.
- Mild redness: It’s normal to experience light redness and a mild gritty sensation as the delicate tissues heal.
- Watery eyes: Some patients may experience frequent tearing for the first few days as the eye heals and adjusts.

When should I be concerned after cataract surgery?
The majority of cataract surgeries proceed without significant problems. However, complications can occur weeks, months, or even years after the procedure, requiring prompt evaluation by a retina specialist.
Some complications include:
- Retinal detachment: Although rare, it is a serious complication that can occur after cataract surgery, especially in patients with high myopia or a history of retinal issues. Warning signs include sudden flashes of light, sudden increase in floaters, and dark shadows in the visual field. Treatment options include laser therapy, vitrectomy surgery, or a scleral buckle procedure.
- Posterior Capsule Opacification (PCO): PCO, often referred to as “secondary cataract,” happens when the delicate membrane situated behind the implanted lens becomes opaque. It can occur months or even years after treatment. PCO occurs when residual lens cells grow on the lens capsule over time, forming scar-like tissue. PCO can be easily corrected with YAG laser capsulotomy, in which a laser is used to create an access hole in the clouded capsule, restoring vision. The procedure takes about 5 minutes and requires no incisions or sutures.
- Cystoid macular edema (CME): Macular swelling can occur days or weeks after surgery. CME causes central vision to become blurry, wavy, or distorted. Treatment options include eye drops, oral medications, and steroid injections to reduce swelling.
- Lens dislocation: Although rare, the intraocular lens can shift from its intended position, causing distorted vision. Depending on the cause, treatment options include lens repositioning or surgery to restore optimal vision.
