What Is Central Retinal Vein Occlusion (CRVO)?
Veins carry deoxygenated blood to the heart. The retina has one major vein called the central retinal vein. CRVO occurs when the main vein that drains blood from the retina becomes blocked. As a result, this vein leaks blood, and fluid collects in the retina, particularly in the macula, which is responsible for central vision. Over time, blood circulation deteriorates, more nerve cells in the eye die, and you lose more vision.
CRVO comes in two types:
- Non-ischemic CRVO: The more common and less severe form associated with leaky vessels in the retina.
- Ischemic CRVO: A more severe type involving significant loss of blood flow, increasing the risk of complications such as glaucoma and neovascularization.
Non-ischemic CRVO can become ischemic, so routine monitoring is crucial to avoid progressive damage to the eye.

What are the symptoms of CRVO?
Symptoms can vary depending on severity, but often include
- Sudden, painless vision loss in one eye
- Blurry or distorted vision
- Dark spots or floaters
- Reduced peripheral vision
Most people develop CRVO in one eye, although it can affect both eyes. The exact cause of CRVO remains unclear. However, diabetes, hypertension, glaucoma, and atherosclerosis have been linked to this condition.
How do you diagnose CRVO?
At Retina Consultants, P.C., we use state-of-the-art diagnostic imaging to assess retinal health. A comprehensive dilated eye exam allows our specialists to examine the retina for hemorrhages, swelling, or abnormal blood vessel growth.
Advanced imaging may include:
- Optical Coherence Tomography (OCT): Provides detailed cross-sectional images of retinal layers to detect swelling and structural changes.
- Fluorescein Angiography: Evaluates blood flow in the retinal vessels and identifies areas of blockage or leakage. FA uses a special dye called fluorescein. Once injected into your arm, it travels through the bloodstream into the eye. Then we use a special camera to take photos of your eye and highlight blocked vessels.
Better diagnostics means better treatment outcomes. Our advanced diagnostic tools allow us to determine whether CRVO is ischemic or non-ischemic and tailor treatment accordingly.
How do you treat CRVO?
The goal of treatment is to stabilize your vision. Treatment is usually performed to seal leaking blood vessels and prevent macular swelling
CRVO treatment options include:
- Anti-VEGF injections: CRVO elevates vascular endothelial growth factor (VEGF). Anti-VEGF medications reduce macular edema and inhibit the growth of abnormal blood vessels. Periodic injections can stabilize or even improve vision in many patients.
- Intraocular steroids or implants: Liquid or small corticosteroid pellets may be used to reduce inflammation and retinal swelling when appropriate. They last several months, but may elevate ocular pressure, requiring eye drops.
- Panretinal photocoagulation (PRP): In cases of ischemic CRVO with neovascularization, laser treatment can help prevent complications such as neovascular glaucoma. Lasers make controlled burns in the retina to reduce bleeding and intraocular pressure.
- Ongoing monitoring: CRVO requires careful follow-up to prevent complications. Our retina specialists develop personalized treatment plans and adjust care as needed to achieve the best possible outcomes.
