PROTECT YOUR VISION WITH HYDROXYCHLOROQUINE-INDUCED RETINAL TOXICITY MANAGEMENT IN CONNECTICUT
Hydroxychloroquine (HCQ) is a widely used medication for autoimmune conditions such as lupus and rheumatoid arthritis. While highly effective, long-term use can, in rare cases, lead to retinal toxicity that may affect your vision. Retina Consultants, P.C. provides precise, proactive screening and expert care to detect even the earliest signs of toxicity, preserving your vision before irreversible damage occurs. As the #1 retina-specific practice serving many locations in Connecticut, we are trusted for our depth of expertise and commitment to excellence.
What is hydroxychloroquine-induced retinal toxicity?
Initially used to treat malaria, hydroxychloroquine is now commonly used to manage conditions such as rheumatoid arthritis and lupus. Although it has proven results, HCQ has ocular side effects. Hydroxychloroquine-induced retinal toxicity occurs when the medication affects the macula, the central part of the retina responsible for sharp, detailed vision.
In the early stages, toxicity may not cause noticeable symptoms. However, as it progresses, it can lead to:
- Difficulty reading or seeing fine details
- Blurred or distorted central vision
- Changes in color perception
- Areas of vision loss
The mechanism of how HCQ causes retinal toxicity is not well known. However, research shows that this medicine affects the metabolism of retinal cells and binds to melanin of pigmented cells, explaining why toxicity persists even after discontinuing use of the medicine.

Who is at risk of HCQ retinal toxicity?
While it’s not possible to predict patients who will get retinal toxicity, high-risk features include:
- A daily dosage of more than 6.5 mg/kg of body weight
- Using medication for more than five years
- Existing retinal disease
- Being aged 60 and above
- Underlying renal or liver disease
- Obesity
Patients taking hydroxychloroquine should undergo routine retinal screening, even if vision feels normal. At Retina Consultants, P.C., our specialists follow advanced screening protocols to identify subtle changes long before symptoms appear. Early detection allows for timely coordination with your prescribing physician to adjust or discontinue medication if needed, protecting your long-term vision.
How do you detect HCQ-induced retinal toxicity?
Detecting hydroxychloroquine toxicity requires highly specialized imaging and expert interpretation. Our practice utilizes state-of-the-art technology, including:
- Optical Coherence Tomography (OCT) for detailed retinal layer analysis
- Visual field testing to assess how different areas of the retina respond to visual stimuli
- Fundus autofluorescence to detect early cellular damage
- Multifocal electroretinography (mfERG) to assess retinal function in different areas
These tools allow us to identify even the earliest signs of toxicity with exceptional accuracy. All patients should have a baseline retinal screening within the first year of starting treatment with HCQ. It should then be followed with annual screenings and thorough risk assessment follow-ups to protect your vision.

Is there treatment for HCQ-induced retinal damage?
There is currently no treatment for hydroxychloroquine-induced retinal toxicity. The best approach is routine screening and stopping medication if at risk. If we detect ocular damage, we may recommend switching to an alternative medicine. Once you cease using HCQ, mild damage may improve. However, patients should know that damage could occur even after stopping medication.
