What is proliferative vitreoretinopathy?
PVR is a severe, vision-threatening condition that involves the growth of scar tissue on the retina. This can cause the retina to become stiff, wrinkled, or pulled away from its normal position. PVR is one of the most common causes of complications following retinal detachment surgery, accounting for over 75% cases. PVR requires a prompt evaluation by a retina specialist to protect vision.

Why does proliferative vitreoretinopathy occur?
Here are two main reasons why PVR occurs:
- Failed retinal reattachment surgery: A detached retina occurs when the retina separates from its underlying support tissue. Retinal detachment can be corrected with surgery, which has an over 90% success rate. Unfortunately, about 5–10% of patients may develop scar tissue after surgery, which can interfere with normal healing and lead to PVR.
- Open globe injury: 50% of patients with an open globe injury are likely to develop PVR. PVR can occur days or even weeks after the initial injury. It is believed that the body’s inflammatory response to the trauma can interfere with proper healing, preventing the retina from fully reattaching.
PVR doesn’t discriminate—it can affect anyone. However, some people are likely to develop it than others. People with diabetes, nearsightedness, a large retinal tear, or complex cataract surgery are more susceptible to PVR. Other risk factors include smoking or having a retinal detachment that involves the macula.
What are the symptoms of proliferative vitreoretinopathy?
The signs of PVR can be similar to those of retinal detachment, including:
- Sudden increase in floaters
- Flashes of light in your vision
- A shadow or curtain over part of your visual field
- Distorted or wavy vision
- Decreased central or peripheral vision
If you experience any of these symptoms, it is crucial to contact us immediately. Early intervention can improve outcomes and reduce the risk of permanent vision loss.

What are PVR treatment options?
Unlike a simple retinal detachment, PVR is more complex due to the scar tissue that prevents the retina from reattaching. It can affect one or both eyes and often requires specialized care from a vitreoretinal surgeon.
Surgical options for PVR include:
- Vitrectomy: A surgical procedure where the vitreous gel is removed and scar tissue is carefully peeled from the retina. This allows the retina to be repositioned and reattached.
- Scleral buckling: The doctor uses a piece of silicone rubber or sponge to hold the retina in place while it heals.
Treating PVR is complex. Sometimes, patients may need 3-4 surgeries for successful treatment. Follow-up care is also critical, as PVR can recur. Close monitoring with your vitreoretinal surgeon ensures the retina stays properly attached.
Are there any non-surgical treatment options?
Yes! At Retina Consultants P.C., we provide the only injection therapy program for proliferative vitreoretinopathy in the state. Methotrexate is an anti-fibrosis medication that has shown efficacy in reducing the risk of recurrent PVR. Our physicians have established an effective treatment regimen that involves specialized in office injections to prevent the need for multiple surgeries.
The use of methotrexate is well-supported through ongoing clinical trials and single center patient analysis (including Retina Consultants P.C. which has published its own success rates using this method to reduce PVR re-detachment!).
