What is vitreomacular traction?

Within the eye, between the lens and the retina, lies a transparent gel called the vitreous humor. As people age, this gel tends to shrink and detach from the retina, leading to a condition called posterior vitreous detachment (PVD). In some PVD cases, however, it doesn’t fully detach and instead remains partially attached to the macula. This creates traction or pulling on the retina, leading to vitreomacular traction (VMT) syndrome.

Over time, this pulling can distort the macula and interfere with your central vision. If left untreated, VMT syndrome increases the risk of maculopathies or disorders, such as a macular hole and epiretinal membranes.

What are the symptoms of vitreomacular traction?

Symptoms of VMT syndrome can vary from mild to more noticeable. You may experience:

  • Blurred or decreased central vision
  • Straight lines appearing wavy or distorted (metamorphopsia)
  • Difficulty reading or focusing on fine details
  • A small dark or gray spot in the center of your vision
  • Seeing flashes of light in the eyes (photopsia)
  • Images appear smaller than their actual size

Because these symptoms can develop gradually, it’s important to seek evaluation if you notice any changes in your sight. Chronic tractional effects can progress to vision loss without proper treatment.

What are the treatment options for vitreomacular traction?

If vision is not affected, your retinal specialist will recommend close monitoring. Sometimes, the vitreous gel continues to shrink and separates on its own without treatment.

Your doctor will recommend treatment if your symptoms worsen or your vision becomes significantly impaired. At Retina Consultants, P.C., treatment options are tailored to your condition and may include:

  • Ocriplasmin (Jetrea®): This injection-based therapy treats certain cases of vitreomacular traction. The injectable enzyme dissolves the proteins that cause the vitreous to stick to the retina, allowing it to separate naturally. This non-surgical option is typically performed in the office and may help relieve traction and improve vision in appropriate candidates.
  • Pneumatic vitreolysis: This treatment involves placing a small gas bubble in the eye to reduce tractional force between the vitreous and the retina. The bubble expands and helps separate the vitreous from the macula over time. It’s a minimally invasive, in-office procedure that can improve symptoms without surgery in select cases.
  • Pars plana vitrectomy surgery: A precise procedure that removes the vitreous gel, releasing traction on the macula. We recommend this treatment for patients with a high risk of significant visual disturbances. Vitrectomy has a high success rate, and patients experience significant stabilization or improvement in their vision.
